 911 Address Application Upon submittal of a 911 Address Application with the $30.00 application fee at the Building/Planning Office, the owner/applicant will be given a piece of white flagging tape to place at the proposed driveway location (either side of an existing proposed driveway or in the center of a proposed driveway not yet cleared). The Planning Official will go to the site and physically measure the location of the driveway and establish the 911 address.

Driveway Permit Application Requires 911 address plaque to be at center of proposed driveway entrance prior to Road Department inspection. Upon inspection, applicant will be notified of required pipe size length

Major Subdivision Application - Private Roads
Temporary Use Permit Application 
(see LDC Sec. 9.14.0.) Must be Approved and issued to applicant a minimum of 15 days before public announcement of the event. Events limited to a five (5) day duration, with limit of 6 events per year /parcel.).

Application for a Major Development Site Plan: Non-Residential Development over 25,000 Sq.Ft. or a Multi-Family Residential Development containing over 10 Units
These require review by the Planning Official, who makes a recommendation to the Planning Commission to approve, approve with conditions, or deny the application. All of these applications require Public Hearings which include providing noticein a local newspaper and certified mail notifications to property owners within five hundred feet (500') of the subject property's perimeter boundary.
Major Subdivision Application - Public Roads
When a Major Development includes dedication of any infrastructure for ownership and/or maintenance by Jefferson County, the development must be reviewed and approved by the Board of County Commissioners to indicate the willingness of the Board to assume ownership and any conditions of ownership by the county, including any maintenance responibilities or other conditions of approval.
Comprehensive Plan Map Amendment 
To change the Land Use District designation of one or more contiguous properties (example: Three landowners with a combined 125 acres wish to change their three contiguous properties from Agriculture 5 to Residential.
Street/Road Name Request/Change Application Form  County Commission approval is only required when requesting the change of an existing name, but not for naming a new public or private road in the county. The application requires three (3) names to be submitted (in order of preference) for review by the Planning Official. Proposed names should not be duplicates or sound close enough to be confused with any existing public or private road in the county (including the City of Monticello). For a rename request, the Planning Official will generally make a recommendation to approve the proposed first choice of the applicants that meets the appropriate eligibility requirements.
New roads are generally created by some form of minor or major subdivision. In some instances driveways may be required to be named as private roads if they serve as access to more than three residences. This is done to ensure all residences and business buildings are assigned unique 911 addresses. The Planning Official will generally approve the name of a new public or private road as long as it will not duplicate, or could otherwise be reasonably confused with, an existing road name. There is no fee for the initial naming of a road.
